Health ePass is a patient held, patient entered record of their health comprising their medical conditions & medications. Additionally it provides an opportunity to record crucial contextual information (e.g. next of kin, mobility etc) & a section for scanned in documents such as hospital correspondence & care plans. It's use puts patients at the centre of their care, allowing them to share their record securely via a simple electronic scanning function, making the information available to clinicians as they choose. It pre-populates clinicians' paperwork, allowing them to verify and amend, rather than collect all the information anew. |

Healthera is a breakthrough innovation designed to help patients and pharmacists manage medications so as to deliver better medicalefficiency and health outcome. The technology currently deployed in some pharmacies in East of England has been well received by pharmacists and patients. GPs are interested in using this platform for better understanding on patient needs and behaviour as well as serving the need for refills. Ambulance and A&E services can be greatly benefitted from quick understanding on patient medication history and behaviour. |

Mologic
Project | Headstart – a new diagnostic for acute exacerbation of COPD |

Description |
COPD is the second most common cause of emergency admissions in the UK, responsible for one in eight acute adult medical admissions. Mologic is developing a urine based test for an early indication of exacerbation, simple enough for patients to monitor their disease status at home. Its design is similar to home pregnancy tests and uses an automatic reader to interpret the results and suggest further management. Providing early recognition of exacerbation, and patient notification to contact their clinician along with clinical information to guide treatment choice, will enable care in the community reducing A&E visits and admissions. |
